I have also been thinking about this recently. And two extensions were written for me for this purpose.
One extension VPN Detector Settings blocks ads if the user uses VPN. The second extension GeoIP AdSense Control blocks ads for selected countries. These extensions are designed to manage AdSense ads. But they will never be presented on this forum.
